09:26: Stations 1, 38, 3 (Ladder), and 99 dispatched to 500 Center Ave, Butler City, for a house on fire with entrapment. One patient is upstairs, inside, with difficulty breathing.
•Control is advising they’re taking multiple calls. The resident/caller is in an upstairs bedroom with the door shut.
09:27: Engine-38, Squad-99, Engine-1-2, and Tower-1 are responding. SQ-99 is requesting air medical on standby. Control is directing crews to the middle bedroom upstairs.
09:28: 99-4 is responding. EN-1-2 is requesting the 2nd alarm.
09:29: Station 3 upgraded to a full-assist; all City shifts recalled to Station 1. EN-1-2 is out with heavy fire showing from the rear.
09:30: Caller is on the floor, laying in front of the window. Second alert for Chief-1.
09:31: Control reports the patient was having increased difficulty breathing and was choking; he or she is no longer responsive.
09:32: EN-38 is to come in nose-first to EN-1-2. Rescue-3-2 is responding. Control advises there is a phone in the room with the patient; they can hear ringing in the background on call back, but the patient is still unresponsive.
09:33: EN-38 is to take a hydrant. LR-3 is on scene. Control advising that the closest hydrant appears to be behind the old St Michael’s School.
09:34: EN-38 is taking the hydrant. Engine-1 is responding.
09:35: Rescue-3 is en route.
09:36: Control is telling EMS units to stage on Ziegler Avenue, per police. The calltaker can hear the FD calling out to the patient.
09:37: “Charge that hydrant.” LifeFlight 4 is on standby with an ETA of 10 minutes to the hospital. EN-1 is arriving on scene.
09:38: Command needs a 2nd handline pulled off of EN-1. “All units, do not call for orders; stage on your line of approach.” Control is advising it sounds like someone is possibly in the room with the caller; they’re stating it sounds like there’s a crew close to the patient.
09:39: A line is being brought in the front door, now. Control is trying to walk crews into the patient.
09:40: Crews are in the room in question.
09:41: Patient extrication is in progress. LifeFlight-4 requested in the air to BMH by SQ-99. Command is now reports the patient has been extricated.
09:46: LF-4 is airborne from the Butler Airport.
09:47: C-1 is on location.
09:49: Lt-1-2 is asking Command to confirm there was only 1 reported victim.
09:50: Command is asking to Control to confirm they’ll have another ALS ambulance on standby when the first transports; 99-8 is in the area, standing by.
09:53: Command is advising Div-2 that they still a decent amount of smoke coming from the eaves and chimney. Interior crews are requesting an attic ladder and hooks. 99-4 is out at the helipad.
09:58: At the 30-minute benchmark, Command is reporting they’re still operating offensively.
10:03: Command is requesting People’s Gas and West Penn Power be notified.
10:04: The address of the incident has been confirmed as 500 Center Ave for utilities.
10:12: Command is requesting the PSP Fire Marshall respond to the scene.
10:13: Control is advising no ETA was provided for the gas company, and the power company will complete a remote disconnect in 3-5 minutes with an ETA of 1-2 hours for a crew. Command is to call in directly to get the FM started.
10:16: SQ-99 reports LF-4 is off the ground, outbound to West Penn Hospital.
10:20: The fire is under control per Command.
10:24: Command is requesting Station 11 respond to the scene to fill air bottles with the cascade system on their Rescue.
10:29: Crews are requesting a chainsaw to the 3rd floor; they have residual between the floors.
10:31: LF-4 is on the helipad at AHN West Penn.
10:32: Rescue-11 is responding.
10:37: Lt-1-2 is advising Command that they’re still chasing a few hotspots on Divisions 2-3. All crews have exited except a few personnel. Command is reporting good smoke venting from Side A Div-2 window; crews are to control interior doors for maximum ventilation. Lt-1 states they’re hitting hotspots in that room right now.
10:40: Command is advising Lt-1-2 that whatever he’s doing, there’s yellowish-brown smoke coming from the window now. Lt-1-2 states they’re opening it up between the floors.
10:47: RQ-11 is on scene.
11:36: LR-3 is clear of the scene.
11:41: EN-38 and RQ-11 are clear.
11:42: 99-8 is back in service, released by Command.
11:44: Stations 1 and 99 are being dispatched for a 2-vehicle MVA, E Jefferson @ 2nd St. One patient with chest pain. EN-1-2 and RQ-1 are clearing the fire to respond to it.
11:45: RQ-3 is clear.
12:51: EN-1-2 is back in service.
12:58: Command is requesting the City PD Detective back to the scene regarding additional footage.
16:01: Chief-1 is the final unit clear of the scene; the incident is terminated.
