MVA: Hooker Rd @ Lou-Lin-Mar Ln

10:00: Stations 25, 14, and 99 dispatched for an MVA with possible entrapment at the intersection of Hooker Road and Lou-Lin-Mar Lane in Concord Township.

10:02: 99-5 and Chief 25-2 are responding.

10:03: C-25-2 is asking if Station 30 has acknowledged and is advised they were not alerted and he is the only acknowledgment so far. He is asking Control to re-tone Station 14 and add Station 30.

10:04: Second alert for Station 14 and Station 30 is now added. 99-2 is also responding. Station 14 acknowledges with 1 firefighter on station.

10:07: Rescue 25 is responding with 1. Second alert for Station 30.

10:08: Control is advising RQ-25 that Station 14 has acknowledged with 1 man and there has been no acknowledgment from 30. Station 31 is now requested.

10:09: Rescue 14 is responding. Chief 31 acknowledges the call.

10:11: RQ-25 is on scene with 2 vehicles on the roadway, 1 flipped over, and a patient out laying on the ground.

10:12: RQ-25 is asking for a helicopter on standby at this time. Squad 31 is responding with a crew of 1. 99-5 is asking if the patient has a head or chest injury. RQ-25 states the patient has a big gash on the top of her head. Per 99-5, they’re to put the helicopter in the air.

10:13: Station 31 units are to setup the LZ.

10:14: RQ-25 is advising 99-5 they’ll probably need a second ambulance and is advised there’s one behind 99-5.

10:15: EN-31 is responding for the LZ. RQ-14 is coming up 308 asking if there is entrapment and is advised negative entrapment. They are to stop at Hazlett Road and shut it down.

10:16: Control is on the line with Stat; they’re the closest, asking where the LZ will be. Command advises the West Sunbury Fire Hall.

10:17: 99-5 is on location.

10:18: 99-2 is instructed to pull in behind 99-5 and is now reporting on scene.

10:19: 99-21 is also on location.

10:20: Per Control, Stat 8 is responding with a 13 minute ETA. RQ-14 is out at the intersection of Hooker and Hazlett.

10:21: Station 25 is advising Command there is 1 man on station if needed to respond.

10:22: Control is verifying that the aircraft is going to Station 25.

10:23: Command now states the new LZ will be Rider Road and Route 38, at the corner. Control states they’ll have to call back to update the LZ. The aircraft is not yet airborne.

10:25: Utility 14 is on scene.

10:26: Stat 8 is airborne from the Grove City Airport. Station 25 is attempting to reach Command again with no response.

10:27: 99-2 is requesting another EMS unit, BLS, for 2 pediatric patients. Station 99 is being asked if they have a unit available.

10:28: Command is asking Control if Stat received the updated LZ and is advised they did, and they are responding to the intersection. Station 99 has a unit available but with an extended ETA. Station 25 is now being advised by RQ-25 to standby at this time.

10:29: Station 52 is being toned out for the call. 99-3 is responding from Butler Hospital.

10:31: Stat 8 is attempting to contact the ground crew.

10:32: A firefighter at Station 25 is advising SQ-31 that Stat 8 is attempting to contact the LZ. 99-3 is en route. Control is also advising Command that the pilot is attempting contact; he is advising the contact will be Portable 25 and Control is instructing them to switch to Air Ops 1.

10:33: The helicopter is about to enter West Sunbury from the west.

10:34: The helicopter is circling over West Sunbury and apparently did not receive the LZ update. Station 25 is attempting to notify Command with no response. The pilot states he is unable to find the scene.

10:35: EN-31 is now acknowledging but Station 25 is apparently not copying them.

10:36: Command is now requesting another ambulance. The helicopter is now headed away from West Sunbury to the south.

10:37: Command is advised both 52-1 and 99-3 were responding. EN-31 is reporting on location at Rider Road.

10:38: Stat 8 is contacting the ground crew and is asking for the LZ briefing.

10:39: EN-31 states they are still working on setting up the LZ now. After the briefing, the pilot states they will be searching. The LZ is to be east of Route 38. The ground crew is asked to advise the pilot of visual contact. The helicopter is now circling east of the Oneida Valley Reservoir.

10:40: Stat 8 is currently near the intersection of Center Drive and Hohn Road and states they are over the last coordinates they were provided and is asking if the ground crew has any visual contact; they do not.

10:41: The pilot is being directed toward the LZ now. 99-5 is transporting to the LZ.

10:42: Commander 31 is directing the pilot toward Route 38; they’re still reportedly way off.

10:43: The pilot states they are at the bottom of the Oneida Valley Dam now. Cmdr-31 is instructing them to head north and is also providing updated coordinates. The helicopter is beginning to follow Route 38 north.

10:44: The pilot states they’re following an ambulance at this time. He is being advised the LZ is now at his 2 o’clock position. 52-1 is responding from Grove City Hospital.

10:45: The pilot reports visual on the LZ.

10:46: The ambulance is to stop at the end of the driveway. Stat 8 is being marked on the ground now. 99-5 is now reporting out at the LZ.

10:47: 99-3 is approaching the scene, requesting orders. They’re coming in from Route 38 and are to pull past the Trooper into a gravel driveway. 99-2 is transporting 2 patients to Butler Hospital.

10:48: Command is advised the Butler Eagle is attempting to get to the scene.

10:51: Command is asking 52-1 their ETA and is advised about 20 minutes.

10:55: Command is advised a Trooper is inbound to the scene.

10:57: Stat 8 is airborne to Presby.

10:59: Command states he is aware 52-1 is still inbound but needs an additional unit possibly from Station 56 or Station 55. Control states as far as they are aware, both 56 and 55 are out of service. Command is instructing Control to send the next closest.

11:00: Squad 31 states he can complete the crew if Control can locate a driver. Command is advised 2 tow trucks are approaching; they are instructed to hold at this time.

11:01: Per Command, the remaining patient will transport by private vehicle; 52-1 is to continue in but the request for an additional unit is canceled.

11:02: SQ-31 is clear from the LZ, responding to the scene.

11:03: Traffic control is now instructed to send the tow trucks up and have them park beside the fire apparatus.

11:04: C-31 is asking if his units are needed at the scene or if they are to go back in service; they are released by Command.

11:05: EN-31 is clear from the LZ and back in service.

11:06: 52-1 is on scene.

11:13: Stat 8 is arriving at UPMC Presbyterian in Pittsburgh.

11:18: SQ-31 is back in service, returning to station.

11:21: 99-21 is back in service from the scene.

11:22: 52-1 is transporting, ALS, to Grove City Hospital.

11:36: Station 14 units are clear of the scene.

11:48: Command terminated per Chief 25-2.
