12:54: Stations 24, 13, and 64 alerted for an MVA with unknown injuries near the Pilot on Route 422 in Muddy Creek Township.
12:55: 64-1 is en route. Stations 24 and 13 OK. Control is advising at least 2 injuries. 64-1 is requesting Station 52.
12:56: Station 52 is alerted. Chief 24-3 is en route to the scene.
12:57: 52-1 is responding.
13:00: Rescue 24 and Engine 13 are en route.
13:01: C-24-3 is on scene with 3 vehicles involved, still on the roadway. Brush 24 is responding.
13:02: 64-1 is approaching from the I-79 side; they are to come up the eastbound lanes and park behind the state trooper. 64-1 is now reporting on location, as well.
13:05: R-24 is on location.
13:06: B-24 is on scene. Command states EMS is requesting a helicopter.
13:07: B-24 is being instructed to pickup another man and then setup a landing zone. E-13 is to pull in behind the ambulance. 52-1 is approaching; Command states the roadway will be closed.
13:08: 52-1 is on location. E-13 is instructed to pull in behind the trooper vehicle as well and is now reporting on location.
13:10: Command states they are going to run one lane of traffic around the scene. B-24 is advising Command the LZ is setup.
13:11: Control states LF-4 will have a 12 minute ETA to Lake Arthur Estates; they are at their base at the Butler Airport currently.
13:16: LF-4 is now departing the airport.
13:18: 52-1 is transporting a patient to Butler Memorial Hospital; 64-1 is transporting their patient to the LZ.
13:19: 64-1 is out at the LZ. The helicopter is crossing 528 south of Prospect.
13:22: The pilot is contacting the ground crew, requesting the LZ briefing.
13:24: LF-4 has the LZ in sight.
13:25: The pilot states the LZ looks good and they’ll be inbound. They were checking more closely due to a reported slight slope.
13:27: The ground crew is now marking the helicopter on the ground.
13:38: B-24, at the LZ, is asking Command if personnel are needed to return to the scene and Command is releasing them.
13:40: LF-4 is airborne en route to Pittsburgh.
13:58: The helicopter is arriving on the helipad at Allegheny General Hospital.
13:59: C-24-3 is terminating the incident; Stations 24 and 13 are clear of the scene.
