15:54: Chief 28-3 is reporting he on the scene of an MVA near 722 Fairview Road. Control states they had a report at 722 N. Washington Road but C-28-3 confirms it is 722 Fairview Road. One occupant is still inside with minor injuries.
15:56: Stations 30, 25, and 56 dispatched to 722 Fairview Road in Washington Twp for a vehicle into a tree.
15:57: Lieutenant 25-2 is responding to his station. 56-2 is en route.
15:59: Station 24 acknowledges the call. Second alert for Station 30. Captain 25 is responding to his station.
16:00: 56-2 is asking for a patient update from C-28-3; he states there was airbag deployment and the patient is bleeding from the lip but appears to have no other injuries.
16:01: Rescue 25 is responding.
16:02: C-28-3 is instructing R-25 to proceed in non-emergency for traffic control.
16:08: 56-2 is on scene.
16:09: R-25 is approaching the scene and is instructed to park in front of the ambulance.

16:44: 56-2 is transporting a patient, ALS, to Butler Hospital.
17:16: Traffic control units are reporting a tow truck has arrived; both sides are instructed to hold all traffic again.
17:35: R-25 is now reporting the incident is terminated, all units are back in service.
