21:03: Stations 14 and 99 alerted for a multiple-vehicle MVA, with injury, near the Sheetz by Clearview Mall.

21:04: 99-2 is assigned.

21:05: Rescue 14, Engine 14 en route.

21:06: 99-2 is responding. Captain 14 is on scene reporting 3 vehicles, off the roadway; he is checking for injury.

21:07: 99-21 is responding as well. Both northbound lanes are to be closed until the Rescue arrives; they have a very large debris field.

21:08: Command is advising 2 patients at this time.

21:09: R-14 is approaching and instructed to redirect traffic into the mall parking lot. E-14 is to setup on the south end of the accident scene.

21:10: 99-2 is approaching. R-14 is on scene.

21:11: 99-2 is on scene.

21:12: 99-3 is also approaching; they are to pull into the Sheetz parking lot.

21:22: 99-21 is asking Control to check the availability of air medical to the BMH helipad.

21:23: LifeFlight 4 will be en route with a 10 minute ETA. The adult patient is reportedly amnestic to the event from high-energy impact.

21:25: Chief 14 is requesting the helicopter divert to the mall parking lot.

21:26: C-14 states he will be the ground contact; Control is advising the helicopter still has the same ETA.

21:29: C-14 is directing E-14 to proceed to the parking lot near Dragon restaurant.

21:35: LF-4 has not departed the Butler airport yet as of this time.

21:37: LF-4 is departing the airport now.

21:40: The pilot is contacting the ground crew for his LZ briefing now, reporting 2 minutes out from the south.

21:42: LF-4 is asking if the patient is on scene and is advised he is there now.

21:44: The helicopter is still circling over the LZ at this time.

21:46: LF-4 appears to be on the ground at the mall parking lot.

21:47: C-14 is reporting the helicopter has landed.

22:01: LifeFlight is reported airborne to AGH.

22:02: Command is terminated; all Station 14 units are back in service.

22:09: 99-2 is now reported clear of the MVA by their dispatcher to handle a medical call.

22:16: LF-4 is on the helipad at Allegheny General Hospital.
